Sample School Behavior Form Extension of the Home Program 

This form is a simplified version of the home behavior chart.  It is designed to make communication easy between parents and teachers and to make it clear what is expected of the child.  It also shows clearly shows each success, so that parents can use the card to substitute for their own observations of the child at school, since they cannot be there.  To download this image, go to the link at the bottom of the page.
